The anesthetic management of tracheoplasty for the stenosis after the tracheostomy: Two cases reports

Abstract

We managed two anesthetic cases of tracheoplasty for the severe tracheal stenosis caused by the cuff of tracheostomy tubes. Dyspnea progressed rapidly and emergency operations were carded out without close discussion with surgeons about airway management and surgical procedure. To insert endotracheal tubes through stenotic lesion was the most difficult problem in these cases, thus, we intubated small size endotracheal tubes without cuff, under the fiberoptic guide technic. Ventilation were done on IPPV without using special technics such as HFJV or CPB. We chose adequate tubes and airway routes in accordance with surgical procedure. Anesthetic managements were performed safely without serious problems. From these experiences we considered that, to assess location and quality of stenotic lesions and to discuss about surgical methods and airway management technics before operation are important for anesthetic management of tracheal stenosis.
